Erin Welsh argues that older reference-range thinking (using a 95th percentile cut-off) led clinicians to treat total cholesterol values around 250 as ‘normal’ in mid-20th-century America. She says heart attacks often occur at total cholesterol levels below those historical thresholds, citing a 200–280 range.
