What happened in this episode?
Actor and filmmaker Joe Manganiello tells Rich Roll that a mysterious autoimmune illness coincided with a major career upswing and the beginning of his marriage, leading to years of severe symptoms, misdiagnosis and escalating medical intervention. He recounts being hospitalised with uncontrolled gastrointestinal bleeding, undergoing emergency surgery in which his large intestine was removed, and living with an ostomy bag while doctors, he says, struggled to provide a unifying diagnosis beyond labels such as colitis/Crohn’s and other syndromes. Manganiello describes secrecy around his condition, including telling media his appendix had ruptured to deter paparazzi after a production shutdown and recasting.
He also details the psychological strain of managing extreme pain treatment—Dilaudid, fentanyl patches and other medication—while maintaining long-term sobriety, and he discloses a period of suicidal ideation in which he gave away a gun. Manganiello alleges an insurance provider later pressured him to stop one of two medications after he improved.
As conventional care offered limited answers, he says he pursued therapy and support groups and experimented with spiritual and alternative practices, including Santería rituals, which he describes in extraordinary terms. A major throughline is ancestry and generational trauma: he discusses Armenian genocide history in his family and says the TV genealogy show Finding Your Roots revealed his paternal grandfather was not biologically related. Manganiello attributes his eventual recovery to a combination of surrender, psychological work, and regaining agency, while stressing he is no longer using painkillers or cannabis and remains committed to sobriety.
